CHARACTERISTICS OF UVEITIS IN SPONDYLOARTHRITIS IN A MULTICENTER STUDY IN MOROCCO

Background Uveitis is the most common extra-articular manifestation of Spondyloarthritis (SpA).

Objectives The aim of this study was to determine the prevalence of uveitis in patients with SpA in a multicenter series and to identify factors associated with its occurrence.

Methods A multicenter descriptive analytical study, including patients followed for SpA in 8 rheumatology departments from 6 different regions in Morocco. All data were measured by standard instruments. Demographic, clinical, paraclinical and therapeutic data of the disease were collected. Disease activity was assessed by BASDAI and ASDAS-CRP scores. Data analysis was done by SPSS.20 software, the p significance level was set at 0.05.

Results The study had included 700 patients followed for SpA, 54% of whom were men. The mean age at diagnosis was 40.42 + 14.19 years. The average diagnosis delay was 59.76 months. The percentage of patients who smoked was 15.6%. A family history of SpA was noted in 11.7%. SpA was non-radiographic in 14% of cases. Bilateral sacroiliitis was described in 49.4%. Coxitis was present in 32.5%. Juvenile SpA represented 15.5% of cases. The prevalence of HLA-B27 was 65.51%. Systemic involvement was dominated by uveitis (11.3%), it was unilateral in 59.5%, bilateral in 28.6% and tilted in 11.9%, anterior in 95.7%. Evolution of the uveitis was acute in 88.9% and chronic in 11.1%. The mean of BASDAI and ASDAS ware 4.42 ±1.67 and 3.19 ±1.24 respectively. The mean CRP and erythrocyte sedimentation rate (ESR) were 35.5 mg/l and 43.3mm respectively.

The mean BASDAI was significantly higher in SpA with uveitis (p=0.025). Biological treatments, in particular tumor necrosis factor-α (TNF-α) inhibitors were more prescribed in patients with uveitis; 47.5% versus 30.9% in SpA without uveitis, with a significant difference (p=0.006). Uveitis was less present in SpA associated with inflammatory bowel disease (p=0.004). However, we did not find a significant association with gender (p=0.61), age at diagnosis (p=0.69), age of onset of symptoms (p=0.8), diagnostic delay (p=0, 58), smoking (p=0.06), clinical forms of SpA (p=0.71), coxitis (p=0.6), HLA B27 antigen (p=0.17), ESR (p=0.56), CRP (p=0.99) and ASDAS-CRP (p=0.51).

Conclusion In our study, the prevalence of uveitis was 11.3%. Its presence was associated with a higher activity according to the BASDAI, and it was associated with a higher use of biological treatments.
